We deliver flexible medical management services that support cost-effective, quality care for our members. As a Health Coach Consultant, you'll be an integral part of our team, working closely with patients and their care teams to improve health outcomes.
This is a remote work-from-home opportunity with virtual training, available anywhere in the US. As a Health Coach Consultant, you'll work intensely as a telephonic nurse health coach, utilizing your clinical expertise and motivational interviewing skills to engage with patients and their care teams. Your role will involve assessing member needs, developing care plans, and collaborating with clinical colleagues to overcome barriers to meeting goals and objectives.
Associate's degree or diploma required; Bachelor's degree preferred.
Our compensation package is competitive, with a typical pay range of $60,522.00 - $129,615.00, depending on experience, education, geography, and other relevant factors. In addition to your base salary, you may be eligible for a CVS Health bonus, commission, or short-term incentive program.
