In this remote position, you will assess the clinical needs of our members, develop and implement care plans, and collaborate with healthcare providers to ensure seamless care coordination. Your expertise and passion for nursing will enable you to make a positive impact on the lives of our members and their families.
Key Responsibilities:
- Conduct comprehensive assessments of members' clinical needs and develop personalized care plans
- Collaborate with healthcare providers to ensure coordinated care and optimal health outcomes
- Utilize clinical judgment to identify and address potential health risks and barriers to care
- Develop and implement strategies to promote member engagement and empowerment
- Participate in case conferences and multidisciplinary team meetings to ensure comprehensive care planning
Requirements:
- Active and unrestricted RN licensure in the state of California
- 5+ years of clinical experience
- Bachelor's degree in Nursing or related field
- Case management experience in an integrated model (preferred)
