Responsibilities
As a Home Health Nurse with Workwarp, you will be responsible for providing high-quality, patient-centered care to patients in the comfort of their own homes. Your responsibilities will include:
- Performing initial nursing assessments on new admissions to identify patient needs and develop a comprehensive plan of care.
- Completing, coordinating, and revising the total plan of care and maintaining continuity of patient care by collaborating with appropriate staff, including physicians, therapists, and other healthcare professionals.
- Regularly communicating patient progress to the clinical manager, physician, and care team to ensure that patient needs are met and that care is delivered in a timely and effective manner.
- Developing, preparing, and maintaining individualized patient care progress records with accuracy, timeliness, and according to care center policies.
- Recording pain/symptom management changes/outcomes as appropriate and making referrals to other hospice care disciplines as needed, or documenting rationale for not doing so.
- Educating families on patient condition, needs, and care to ensure that they are empowered to provide optimal care and support to their loved ones.
- Performing on-call responsibilities and on-call services to patients/families as assigned, including responding to emergencies and providing crisis intervention as needed.
- Performing other duties as assigned by the clinical manager or other members of the care team.
Qualifications
To be considered for this role, you must have:
- A current RN license in the state of Pennsylvania.
- At least one year of nursing experience, preferably in a home health or hospice setting.
- A current CPR certification.
- A valid driver's license, reliable transportation, and liability insurance.
Preferred Qualifications
While not required, the following qualifications are preferred:
- Experience working with patients with complex medical needs, including those with chronic illnesses, disabilities, or terminal conditions.
- Knowledge of hospice care principles and practices, including pain/symptom management and end-of-life care.
- Experience working with interdisciplinary care teams, including physicians, therapists, and other healthcare professionals.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
