As a Home Health Aide, you will play a vital role in coordinating and providing care to our hospice patients, addressing their personal care needs, and promoting their comfort and well-being. Key Responsibilities:
- Coordinate and provide care to hospice patients, including personal care, emotional support, and comfort measures
- Calm, soothe, and empathize with patients and families, addressing their strong emotions and concerns
- Participate in interdisciplinary team meetings, collaborating with healthcare professionals to deliver superior patient care
- Assist with observing patients, documenting care, and reporting any changes or concerns to the healthcare team
- Collaborate on cases, participate in training and development, and engage in other organizational activities as required
Your Experience and Education
To be successful in this role, you will need:
- A Certified Nursing Assistant Certificate issued by the State Department of Public Health (required)
- A Certified Home Health Aide certification issued by the State Department of Public Health (preferred)
- Current BLS/CPR certification
- At least one year of aide experience, with one year of hospice experience preferred
- Possession of a valid California Driver's License, access to a reliable automobile, and minimum car insurance coverage
- Excellent observation, problem-solving, verbal, and written communication skills
- Demonstrated ability to efficiently use a computer, with prior electronic medical record (EMR) experience preferred
