A Care Coordinator supports our growing patient population. This job involves onboarding new and expecting parents, gathering information for appointment booking and insurance verification, and helping patients stay involved and engaged throughout the pregnancy, birth, and postpartum journey.
The goal of this role is to promptly onboard all patients seeking care from Mahmee and match them with the appropriate line of care from our team with the highest level of compassion and customer service.
You will need experience and/or knowledge about:

Enrolling Patients and Receiving ReferralsAnswering/placing calls from/to patients and providers that promote trust, respect, inclusion, and empathyGathering the information needed to onboard new patientsPrompt follow-up with patients and providers to close the loop on outstanding questions and concerns Scheduling
Scheduling appointmentsTriaging patients and matching the patient to the appropriate type of care and time on the calendarDocumenting patients' needs and any relevant information for the scheduled provider Escalation of Concerns
Prompt communication about patients' needs to the Mahmee teamProcessing physician’s orders; being in contact with outside physicians and referral partners to provide status updates on delivery of care and patient statusCompleting handoffs of any patients or action items to other team members to ensure continuity of care when your shift ends Coordination

The position will typically be from Monday to Friday, 8:30 AM to 5 PM or 9 AM to 5:30 PM, with one monthly weekend rotation. Occasionally, you will be scheduled to work on recognized holidays.
