High Risk Maternal Health Case Manager

Job Description Molina Healthcare Services is seeking a skilled High Risk Maternal Health Case Manager to join our team. As a key member of our case management team, you will play a critical role in ensuring the health and well-being of our high-risk pregnant members. The ideal candidate will have 1-3 years of experience in case management, disease management, or medical/behavioral health settings. You will possess strong clinical assessment skills, with the ability to determine who may qualify for case management based on clinical judgment and changes in the member's health or psychosocial wellness. Key Responsibilities: • Develop and implement individualized case management plans in collaboration with the member, caregiver, physician, and other healthcare professionals. • Conduct regular outreach and monitoring to evaluate the effectiveness of the care plan and make adjustments as needed. • Promote integration of services, including behavioral health care and long-term services and supports, to enhance the continuity of care for Molina members. • Facilitate interdisciplinary care team meetings and informal ICT collaboration to ensure seamless communication and coordination among healthcare professionals. Requirements: • Completion of an accredited Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) Program • 1-3 years of experience in case management, disease management, managed care, or medical/behavioral health settings • Valid driver's license with good driving record and reliable transportation Preferred Experience: 3-5 years of experience in OB High Risk/maternal health case management, disease management, managed care, or medical/behavioral health settings Benefits: Molina Healthcare offers a competitive benefits package, including a comprehensive salary range of $24 - $56.17 hourly, depending on location, work experience, education, and skill level.
